Clinical Profile of Gastroesophageal Reflux Disease (GERD) with Helicobacter pylori Infections

ABSTRACT

Background: Over the prior decade, there has been a significant increase in the proportion of younger patients with Gastroesophageal Reflux Disease (GERD), specifically those within the age range of 30–39 years. Several pathophysiological factors influence the development and the course of gastroesophageal reflux disease (GERD). Helicobacter pylori infections, the most common human chronic bacterial infection, modify gastric acid secretion, simulating gastroduodenal disease pathogenesis, including GERD. Our study aims to investigate the clinical profile of GERD with Helicobacter pylori infections.

Methods: A retrospective analysis of medical records taken from GERD patients diagnosed with Helicobacter pylori infections between 2014-2018 in Wahidin Sudirohusodo Hospital. Data about patients’ demography, symptoms, lifestyle, GerdQ Questionnaire score, and endoscopy findings were collected.

Results: A total of 159 medical records were analyzed, <50 years old 107(67,3%) samples and ≥50 years old 52(32,7%) samples. We found 86(54,1%) samples were men, and 73(45,9%) samples were women, with a mean BMI of 20,97. Based on GerdQ Questionnaire 92(57,9%) patients categorized as GERD and 67(42,1%) Non-GERD patients with 152(95,6%) grade A, 6(3,8%) grade B, and 1(0,6%) grade C. The symptoms were 88(55,3%) epigastric pain, 86(54,1%) nausea, 112 (70,4%) bloating, 29(18,2%) acid regurgitation, and 22(13,8%) heartburn. The most common endoscopy finding was gastroduodenitis 68(42,8%). From the data, we also found that 150(94,3%) and 125(78,6%) patients did not smoke or drink alcohol and consumed fewer fats 107(67,3%) samples and fibers 85 (53,5%) samples.

Conclusion: The major profile of GERD patients with Helicobacter pylori infections in Wahidin Sudirohusodo Hospital was dominantly man, <50 years old, mean BMI 20,97, LA classification grade A, non-smoker nor alcoholic, consuming lower fibers and fats, with epigastric pain and gastroduodenitis as the most common symptoms and endoscopy findings respectively.
