Relationship between age, symptoms, and level of stomach damage in patients with NSAID Gastropathy Diagnosis in Saiful Anwar Hospital, 2019-2020

Abstract

Background: Many recent studies have shown a significant increase in the incidence of drug-associated gastrointestinal (GIT) lesions. Most GIT lesions are associated with the use of non-steroidal anti-inflammatory drugs (NSAIDs), especially during uncontrolled use. NSAID gastropathy and gastrointestinal (GI) bleeding are severe side effects of NSAIDs. It is important to be able to know the characteristics of NSAID gastropathy and to know the early signs of this disease so that it can provide prevention and treatment from the start.

Methods: This study is an observational study with cross-sectional data collection to determine the relationship between age and symptoms with the level of gastric damage in patients with NSAID gastropathy who underwent endoscopy at the RSSA from January 2019 to December 2020. The research subjects were all patients with complaints of dyspepsia with a history of NSAID consumption. who underwent endoscopy were included in the inclusion criteria. The exclusion criteria from the study were other diseases on endoscopy such as esophageal varices, gastric congestion, or gastric masses.

Results: Of the 626 patients who underwent endoscopy, 392 patients had gastritis (51%). Of the 392 gastritis patients, 90 had NSAID-induced gastropathy (14% of all endoscopy patients). The probability value (p value) of the correlation of age with symptoms, age with the level of gastric damage, and symptoms with the level of gastric damage, respectively, was p = 0.018, p = 0.094, and p = 0.140, respectively.

Conclusion: The incidence of NSAID gastropathy is quite high in patients with endoscopy at RSSA, with a mean patient age of 52 years. There is a significant relationship between increasing age and symptoms of NSAID gastropathy. There was no significant relationship between age, symptoms, and the level of gastric damage.
